The plan is underwritten by Medical Associates Health Plans which has over 35 years of expertise insuring members in multiple states in the Midwest. Live360 Health Plan features providers from HSHS Medical Group, Prairie Cardiovascular, Springfield Clinic, SIU Medicine, and many other primary care and specialty providers in the area.

Live360 (underwritten by Medical Associates Health Plans) operates in ten counties in south central Illinois. This network provides access to HSHS Hospitals, located in Decatur, Effingham, Springfield, O’Fallon, Litchfield, Highland, Greenville, Shelbyville and Breese.
In addition, the network includes the practitioners of HSHS Medical Group, Prairie Cardiovascular, as well as the practitioners affiliated with more than 40 independent physician groups including Springfield Clinic and SIU.
